Purpose. Workers complete this form before any non-routine or high risk task to break the job into steps, identify hazards and agree controls with the crew.

1.When a job safety analysis is required

A job safety analysis must be completed for any task that is not covered by an existing procedure, any task in a public place, any chainsaw or tree work above shoulder height, any work near overhead or underground services, any chemical application near sensitive areas and any task the supervisor rates as high risk. The completed form must be signed by every crew member before work starts.

2.Breaking down the job

List the job in the order it will be done, in steps small enough that each has its own hazards. For each step identify what could go wrong, who could be harmed and how badly. Rate the risk before controls using the Greenfield risk matrix.

3.Choosing controls

Choose controls from the top of the hierarchy where possible: eliminate the hazard, substitute a safer method, isolate people from the hazard with barriers or exclusion zones, then administrative controls and finally personal protective equipment. Rate the risk again with controls in place. Any step still rated high must be referred to the supervisor before work starts.

4.Briefing the crew

The crew leader must walk the crew through the form on site, confirm the emergency plan, first aid and communications, and have each person sign. Any change to the job during the day requires the form to be reviewed and re-signed.

5.Filing

Completed forms must be attached to the work order in the Work Orders system or filed with the day's paddock record, and retained for two years.

6.Complete the form
